<![CDATA[Step 1: Likelihood]]>

Use of the Risk Assessment Matrix occurs over three steps. In the first two steps, the Likelihood and Consequence tables help to determine their scores. In the final step, the Risk Assessment Matrix cross-references these tables to give an overall risk score (1–25) and level (Low, Medium or High).

Consequence

5	Catastrophic	Death or permanent disability preventing any return to work
4	Major	Serious injuries or health problems requiring hospitalisation and a significant period of rehabilitation before being able to recommence work
3	Moderate	Injuries or health problems requiring medical attention and a short rehabilitation period before being able to recommence work
2	Minor	Injuries or health problems requiring local first aid and no rehabilitation period
1	Insignificant	No personal risk and no injury
